1. Begin with a new file, size and colors doesn't matter. Then create a new layer. Now take the Rounded Rectangle Tool and draw out a shape similar to the one below.
2. Then go to the layers palette and right click on Layer 1, choose Blending Options and apply these settings.
3. Click OK, and you should have something like this.
Then add some text, and maybe inset lines, and you're done! This is what I came up with.
