1. Begin with a new file, 400x400. Then create a new layer in the layers palette and then take the Polygonal Lasso Tool and try to draw a shape similar to the one below.
2. Now head over to the channels palette and click on the Save selection as channel icon. Then click on the Alpha 1 channel, then press Ctrl+D to deselect. Now go to Filter>Blur>Gaussian Blur and set the Radius to 4 pixels. And then press Ctrl+L and apply this settings.
3. Then click on the Load channel as selection icon, then go back to the layers palette and click on layer 1, now right click inside your shape and choose Fill, I use #003313.
4. Now when you have your shape selected create a new layer, and then go to Edit>Stroke and set the Width to 1px, color black and Location Center. Then go back to layer 1 and apply this settings.
Now should it look something like this.
5. Create a new layer and then move that layer below layer 1. Then once again take the Polygonal Lasso Tool and make a shape similar to the one below.
6. Then fill that shape with the same color you have on your larger shape. Then repeat step 4. And now should it look something like this
7. Create a new layer and then move the layer above layer 1. Now take the Pencil Tool and make sure you have black as your foreground color, then draw from left to right, hold down Shift while drawing to get a straight line. Then duplicate the layer and change the color.
Then repeat step 7 and use the Move Tool to move those lines to the bottom of your content box. Then add some text and you are done! This is what I came up with.
