Step 1:
Let's start out by creating a new file. I used a 600x200 pixels canvas set at 72dpi, and I filled my background with black. Now make a new layer set and name it 'Text Design'.
Select the Horizontal Type Tool then set the font family to Bookman Old Style, bold, 72 pt, strong and #70D909 color shade. Now create a new text layer then type your name.
Step 2:
Under Layer Style(Layer > Layer Style) add an Inner Shadow and Inner Glow blending options to your green text layer.
Step 3:
In a new text layer type your name in white color shade.
Step 4:
Under Layer Style(Layer > Layer Style) add an Inner Shadow, Inner Glow and Pattern Overlay blending options to your white text layer.
Step 5:
Duplicate your 'Text Design' layer set then go to Layer > Merge Layer Set. Then under Layer Style(Layer > Layer Style) add a Gradient Overlay blending option to the Text Design layer. Nudge it down by a couple pixels as shown below.
Step 6:
Move the Text Design layer behind all your text layers to create a 3D effect.
