Start with a new document. Size 600x600 Pixels with a white background
Create a rectangle using rectangular shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Press Ctrl+T. Right Click and select Warp. Select "Arch" from the preset warp shapes. Adjust settings as shown.

Open a new document in photoshop with a grey background

Then Select rectangle tool and create a shape ( hold the Shift key when you are drawing the shape - in this way your shape will be a perfect square)
Open a new document 500x500 pixels with a white background
Then create a new layer. ( Press Shift+Ctrl+N )
Select Rectangular Marquee Tool (M) and make a selection like mine:
