1. Open a picture you like, I chose this one from a beautiful girl.

2. Pick the Lasso Tool(L) and make a shape like the picture

3. You'll get a selection. Create another layer, call it "blood", and fill this selection (alt+f) with a red color #A90101. Then, change the blood layer to Color Burn mode, and it's opacity to your taste..

4. Press D (for the Foreground color be Black) and with the Burn Tool make the corners of the shape darker. Now with the smudge tool (R) in some size between 9-14, depending on the size of your image, and with the strength of 30%-75%, apply it to the blood so it will look like this:

5. Now go to Filter > Texture > Texturize and apply the Burlap texture, play around with the settings, I used 200 px. Duplicate the "Color Burn" layer and put it as Overlay. Play with both layers opacity, until you find a variation you like (this depends most on the lightness of the picture you are using)

You are done!
