We'll begin this tutorial, by creating a new canvas, with a size of 340 x 184, then draw out a polygon shape using the "Polygon Tool" with the below settings. Make sure black is the canvas color.
You should now have the following image.
You will now have to make a diamond. You candownload the diamond effect here. Once opened, Simply open your "Styles Palette" and apply the style.
Now right click on the polygon layer and select "Blending Options...Now click on "Drop Shadow".
Do not click ok yet...still much more to do.
Now click on "Pattern".
You may now click "OK."
You should now have the following image.
Next, duplicate the polygon layer and then go to the main menu and select "Edit" then "Free Transform". Decrease the size approximately the same as the below image. Then clear the layer style by going to the layers palette menu found in the layers palette and make sure the color is black.
Now right click on the polygon layer and select "Blending Options...Now click on "Stroke".
You may now click "OK."
You should now have the following image.
Next, duplicate the polygon layer once again and then go to the main menu and select "Edit" then "Free Transform". Decrease the size to approximately 2% and use the color 082542. Clear the layer style once again.
Now right click on the polygon layer and select "Blending Options...Now click on "Inner Shadow".
Do not click ok yet...still much more to do.
Now click "Outer Glow ".
Do not click ok yet...still much more to do.
Now click on "Inner Glow".
Do not click ok yet...still much more to do.
Now click on "Satin".
Do not click ok yet...still much more to do.
Now click on "Gradient Overlay".

You should now have the following image.
Next, add a letter as I have illustrated below using the color 939fb9. To create the "M" I used the "Pen Tool".
Now right click on the text layer and select "Blending Options...Now click on "Drop Shadow".
Do not click ok yet...still much more to do.
Now click on "Outer Glow".
Do not click ok yet...still much more to do.
Now click on "Inner Glow".
Do not click ok yet...still much more to do.
Now click on "Bevel and Emboss".
Do not click ok yet...still much more to do.
Now click on "Gradient Overlay".
You may now click OK.
