Create a rectangle using rectangular shape tool.

Enable subtract form shape area form the top bar.

Create two ellipses using elliptical shape tool on both sides.

Enable add to shape area form the top bar. Create an ellipse using elliptical shape tool.

Double click the layer to open layer style window. Apply layer style with the settings shown.

Create a rectangle using rectangular shape tool in side the main header. Enable add to shape area form the top bar. Create an ellipse using elliptical shape tool.

Enable add to shape area form the top bar. Create a rectangle using rectangular shape tool. Using direct selection tool adjust the two top nodes of the rectangle as shown.

Double click the layer to open layer style window. Apply layer style with the settings shown.

From layer menu select layer style>>create layers. It will split layer and layer style in separate layers. Select the layers and merge.

Press Ctrl+T. Right click the layer and select warp.

Select Arc from presets.

Similarly create another thin rectangle and apply steps explained above to convert it to a 3d pipe.

Create and rotate small rectangles and place as shown in the image below. Apply similar gradient overlay and stroke layer styles as explained above.

Select the main shape layer that is lower most in layer order. Select the path using direct selection tool. Select convert point tool and click on top node as shown.

Create a small triangle using custom shape tool.

Apply bevel & emboss layer style along with stroke to give it a 3d look.

Duplicate the triangle several times and place along with button links etc.

Place company logo at top of the header.
