2. Create a new layer in the layers palette, then take the Rectangular Marquee Tool and draw out a shape as the image below shows and fill it with black.

3. Then go to Filter>Noise>Add Noise and set the Amount to 40, Gaussian, Monochromatic. then go to Filter>Blur>Motian Blur and do this settings, Angel 0, Distance 980. Then press Ctrl+U and set the Hue to 220, Saturation 75, Lightness 10.

Now should it look like the image below shows. ( Click for larger view )

4. Create a new layer and take the Rectangular Marquee Tool and draw out a shape as the image below shows and fill it with black.

5. Then go to Layer>Layer Style>Bevel and Emboss and do this settings.

6. Then duplicate that layer, to duplicate just right click on the layer and chose Duplicate Layer. Then take the Move Tool and move that part to the bottom of the header.

Now should it look like this.

7. It´s time to make the menu, once again create a new layer and take the Rectangular Marquee Tool and draw out a shape and fill it with #2B2B2B, then take the Text Tool and add some text to it.

Now should it look like this.

8. Create a new layer, then take the Rounded Rectangle Tool and draw out a shape as the image shows.

9. Then right click the layer and chose Blending Options and apply this settings.

Now should it look like this.

Then for the left side just repeat step 8 and 9.

And you are done!
