1. Create a new file (780x100), Background Contents: Transparent
2. Press D to set Foreground color (White/Black)
3. Use Filter -> Render -> Clouds
4. Use Filter -> Render -> Difference Clouds , Press Ctrl + F more time (4 or 5 times)
5. Next, Press Ctrl + I to invert color
6. User Image -> Adjustment -> Levels (or Ctrl + L). Set Middle point as 0.2
7. Press Ctrl + U ,check Colorize and choose 1 color that you like. I like Blue, so I set :
Hue: 203
Saturation : 25
Lightness: 0
8. Duplicate layer (Ctrl + J)
9. Select the layer on top. Use Filter -> sketch -> Chrome . With default parameters :
Detail : 4
Smoothness: 7
10. Use Blend Mode : Soft light in this layer (or Right Click -> Blending Options -> Blend mode)
11. Duplicate Layer (Ctrl + J)
12. On the top Layer, Use Filter -> Pixelate -> mosaic, Set cell size = 12
13. Filter -> Brush Strokes -> Accented edges
Edge width : 2
Edge Brightness: 50
Smoothness : 5

15. User Regtangular Maquee Tool create area , fill by White (foreground: White, press Alt + Backspace) , like this
16. Press Ctrl+A to select all, CTR + C to copy , to come back previous layer, Ctrl + V to paste
17. Blending Options -> Stroke. Set : size = 1, Color = Black , Position = Center
18. Set Optical = 50 percent
19. Use Horizontal Type tool and type a text you like
