With the "Custom Shape Tool" stil selected, choose the "display" shape and draw it as I have done below. Name this layer "Display". The color of this layer does not matter.
Now right click on the "Display" layer and select, "Blending Options...". Now click on "Satin".
Do not click "OK" yet. Still more to do.
Now click on "Gradient Overlay".
Do not click "OK" yet. Still more to do.
Then click on "Stroke".
You may now click OK.
You should now have the below image.
Now select the "Brush Tool" and choose 0457fe as the color. Create a new layer and name it "Lower Highlight" then change the Blending Mode to "Color Dodge".
Next, "Ctrl" click the "Display" layer to create a selection around it. Make certain you are on the new layer "Lower Highlight" and begin to brush along the lower inside part of the selection as I have done below.
Next, change the color to "0a499f" and create yet another layer and name this one "Upper Shadow". Change the Blending Mode of this layer to "Mutiply" then begin to brush along the top inside part of the selection as I have done below using the same brush settings as was used above.
With the selection still made, create another layer and name this "Gloss" and change the foreground color to White. Go to the main menu and choose "Select" then "Modify" and "Contract" by 2.
Now choose the "Eliptical Marquee Tool" and while holding in the "Alt" key on your keyboard draw an oval shape to deselect the lower portion of the selection. Using the spacebar on your keyboard will let you move the selection around as you draw it.
Once satisfied with the selection, Select the "Gradient Tool" and change the gradient to "Foreground to Transparent" in the options palette. Then, starting at the top of the selection, click and drag to the bottom of the selection and release.
You should now have the below image.
Next, create your display text and such. For the bars (equilizer), you can simply make a lot of little White rectangles. Experiment with it.
Next, choose the "Custom Shape Tool", find the "menuframe" shape and draw the shape as I have done below. Name this layer "Menu Frame" in the Layers palette.
Now right click on the "Menu Frame" layer and select, "Blending Options...". Now click on "Gradient Overlay".
You may now click OK.
You should now have the below image.
Next, draw the same shape as above but in Black this time and a little smaller than before as illustrated below.
I've added a gloss layer, as you can see by using the same process as in step 27 above. Instead of using a gradient however, I filled with the color 4a4a4a on a new layer.
Next, select the "Text Tool" and choose Arial Bold as the typeface at a size of 8 and color of 959595. Type your menu items and go to next step.
Now, select the "Line Tool" and draw some divider lines using Black as the dark lines and 838383 as the light lines.
We're almost there.
