Start by opening an image of a lady's ear in focus.
Create a flower shape using custom shape tool.
Rasterize this layer. Enable lock transparency pixels and with a small feathered brush create some black spots on it.
Form filter menu select>>distort>>glass. Apply settings as shown.
Apply following layer style to create a golden frame for the diamonds.
Scale down the ear ring so that it hangs nicely on the ear.
Apply following layer style to create a upwards shadow of the ear ring.
Duplicate, scale down and place below the original one. Reduce the stroke width.
Similarly create another instance of this layer.
Add some sparkles by using sun shape from custom shapes.
