1. Create a new layer 2 by 2 pixels in size. Fill one pixel in black (this will be the color of the perforation).
Then select Edit > Define Pattern. Name your pattern and click okay.
2. Discard the old file and open a new file sized 100 by 100 pixels.
Create a new layer and call it grid. Fill this layer with the pattern by selecting the paint bucket icon, then select "pattern" as the fill type in the options window.
Select the pattern we made in step one and fill the layer.

3. Now make a circle selection, roughly in the center, by holding down shift and using the circle selection tool (
). Now goto filter > distort > spherize .
Now inverse the selection by going to select > inverse. Then expand the selection by going to select > modify > expand. Type in 10 then press delete to clear the selection.
4. Now to apply some styles. Right click on the "grid" layer and select Blending options Select Bevel and emboss. Set the size to 50 and soften to 16.
Set the highlight opacity to 100%. Set the shadow opacity to 40%.
Also apply a contour by ticking the contour box.
5. In the blending window add a stroke of black (#000000) and set the opacity to 50%
Finished. Hope that wasn't too hard to follow. You could modify this tutorial to make golf balls. (i might do it later). To the right is an example image made using the tutorial, makes a nice button. All that was added was an overlay of color, an outer glow and some text with a slight drop shadow.
