Open a new document with a black background ( 400 x 400 pixels will be fine )
Type some text and add the following layer styles:
Let's add a layer style for this white rectangles:
This is the result:
Hold the Ctrl key down and press on the layer thumbnail
You will notice that you have selected all the text
Grab the Rectangular Marquee Tool
Select " Subtract from selection " and make a selection like in the next image
When you release the mouse button you will have only the top part of the text selected
Go to Select > Feather or press Ctrl+Alt+D
Change the feather radius to 5 pixels and press OK
Move the selection with the up arrow key
Create a new layer ( press Ctrl+Shift+Alt+N ) and fill your selection with white Then press Ctrl+D to deselect your selection
Now add the following layer styles
This is the result:
If you want to add more details duplicate this layer ( press Ctrl+J ) then change the Blending Mode , Opacity and Fill like in the next image
