1. First thing you're going to need to do, is start a new document:

image 1

2. Use Control+A to select all, then Control+I to invert all the colors. Your background should now be black.

3. Write your name/text centered on the canvas. It doesn't need to be exact, it just needs to fit with a little bit of extra space on all sides. Font and size don't matter as long as the font isn't too curvy or complex, and it fits on the canvas. If it doesn't fit, use a smaller font or bigger canvas. I used Arial Black for font, and 72 for size. More importantly... Your text must be white.

It should look similar to this so far:

4. Copy the text layer twice, giving you a total of 3 of the same text layer. You can copy the layer, by clicking and dragging on the text layer in the layers panel(which can be found in the lower right corner), and dragging it into the small icon left of the trash can, also known as the new layer icon.

Your image should look essentially the same as it did before.

5. Rasterize the 2 copies of the text layers, by clicking right on the layer in the layer tab, and clicking "rasterize type".

6. Make sure the first copy of the text layer is your active layer, and use Filter -> Blur -> Guassian Blur. Use a radius of 26 pixels.

Your image should look similar to this:

7. Make the second copy of the text layer your active layer, and use Filter -> Blur -> Motion blur. Use the following settings:

Angle: 90

Distance: 134 pixels

Your image should look similar to this:

8. Rasterize the original text layer by clicking right on it, and clicking "Rasterize Type".

9. Make sure the original text layer is still your active layer, and use control+A to select all, and control+I to invert.

Your image should now look like this:

10. Go to your layers tab, and drag the original text layer ontop of the 2 text layer copies, like so:

11. Your image should now look like this:

12. Now click right on the farthest-bottom layer, also known as the background layer, and click right, then click "Flatten Image", to merge all layers together.

Your image should look the same as it did at step 11.

13. Now use filter -> blur -> Guassian Blur. Use a radius of 2.5 pixels.

Your image should now look like this:

14. Now use Image -> Trim. Use the following settings:

Your image should now look like this:
