Open a new document with transparent background. I've chosen 200x100 px. Create a new layer and set the foregroundcolour to #A2ACA3 and grab Rounded Rectangle Tool. Set radius to 5 px in Options Bar and make a rectangle.

Go to Filter > Noise > Add Noise

Amount: 5

Uniform: X

Monochromatic: X

Go to Inner Shadow (blending options) - leave the settings and go to Bevel And Emboss where you set Style: Pillow emboss

Hit OK.

The font I used is named DS-Digital - DaFont

Hit D on the keyboard and pick the text-tool and type "88:88". Create a new layer and type "02:45" or something like that. Grab Move Tool and set the text-layers right over each other. Remember that "88:88" is placed under "02:45" in layers-palette

You are finish.
