1. New document in Photoshop, I am using 340x200, but you can make yours a little larger. Also, add a black background.

2. Create a New Layer.

3. Using the Polygonal Lasso tool and make an interesting shape like mine, and fill it with a midgrey like #7F7F7F.

4. Go to Select > Modify > Contact and I used a value of 12px.

5. Now press Delete.

6. Press Ctrl + D to deselect.

7. Using the Dodge tool (Press O), lighten up the corners of your grey border.

8. Now select the Burn tool, and darken up the straight parts to give your border a gradient effect.

9. Right click on Layer 1 (the border layer), and select Blending Options and use these settings.

Drop Shadow - Blend mode: Multiply; Opacity: 75%; Distance: 5; Spread: 0; Size: 5.
Inner Shadow - - Blend mode: Multiply; Opacity: 75%; Distance: 5; Spread: 0; Size: 5.
Outer Glow - Blend move: Screen; Opacity: 43%; Colour: #FFFFFF (white).
Bevel - Style: Inner Bevel; Size: 5; Soften: 0.
Contour - Range: 50%.
Gradient Overlay - Blend move: Multiply; Gradient: Black to white; Scale: 70%.
Stroke - Size: 1px; Colour: #000000 (black).

10. Press Ctrl + J to duplicate the layer, now move it up and left a little. Make the layer underneath a little lighter so it is more visible too.

11. There you have it! Now just add your own text, and maybe some images or scanlines to the inside of it. Here is my final, looking good!
