Step 1:Open up a new image with Width=700 and Height=1000, this will be your whole template. Make the background any color you want.
Step 2: Make new layer and using the rectangle tool(press "U" on keyboard), make a square block at the top left corner like this. Hold "Shift" to get a perfect square.
Step 3: Now using the elliptical marquee tool(press "M" on keyboard), make a circle twice the size of the square block. Now move the circle over the square like so...
Now hit the "delete" button on the keyboard and you should end up with something like this.
Then go to Select> Deselect, to deselect.
Step 4: Duplicate layer 2, the layer with the shape you just made and flip is by going to Edit>Transform>Flip horizontally. Move the duplicated layer to the opposite end of the header, using the move tool.
Step 5: Press Ctrl+E to merge down the 2 shape layers. Now add connecting bars using the rectangle too(U), to connect the 2 shapes.
Step 6: Now to add the layer style to the header, Go to Layer>Layer style>Blending options and insert these settings.
Step 7: Now to make minor details like punching holes in the header.Use the elliptical marquee again to make two small circles on each side(hold shift), one at a time. Then hit delete.Then add your banner picture under the header layer.
Step 8: Make a new layer under the header layer and add 2 small shapes in the 2 holes, using the custom shapes tool. I used this shape.
Step 9: To make the glass effect make a new layer then make a white long bar using the rectangle too like so.This layer must be over the banner but under the header.
Step 10: Using the Elliptical marquee tool make an oval like shape.This is what its suppose to look like.
Hit delete
Step 11: Go to Layer>Add layer mask>reveal selection and the using the gradient tool on default settings, go down to up like this.
Once you're done, click once on the layer thumbnail then use the eraser tool to erase the glass layer from the 2 holes and you're finish at last. Added a little final touches.
